As a New Patient, if you want to fill out your Patient Registration forms in person, please plan to arrive 45 minutes before your scheduled appointment. Unfortunately, with respecting everyone's schedules, we may have to reschedule your appointment if you arrive at your scheduled time, with no paperwork. **Our office is in-network with most plans but not all. We are out of network with: Bright, Cigna, Friday, Humana, Kaiser Permanente, and Medicaid/Health First Colorado. We are happy to accept out-of-network benefits' usual and customary charges. We also have a credit card on file policy.
